Your answers allow Ebando to check that your coming is appropriate, to anticipate any contra-indications, and to welcome you with full knowledge of the situation. No data is sent or stored by our site. Everything stays saved in your browser, on this device, until you pass the document on to the association yourself.

Please also tell us of any illness, or any pathological or functional issue — physical or psychological — not covered here.

Once the questionnaire is filled in
How to pass your document on to Ebando.
Go through the seven sections, sign the declaration at the end, and tick the documents you will attach.
Open the summary, then click “Print / PDF” to save the document as a PDF (or print it).
Send this PDF to Ebando yourself, by WhatsApp or by email to OngEbando@gmail.com.
Attach your documents: a recent passport-style photo, and a medical certificate confirming the absence of cardiac, gastric and psychiatric conditions.
Privacy
Your data never leaves your device.
- The form does not process, send or store any data on the site side. Everything is saved locally in your browser.
- You stay in control of the document: it only leaves when you pass it on yourself, by WhatsApp or email.
- Once received, your answers are reviewed with you by a single person at Ebando — never by the whole team — and are never published.
- You can erase everything from this device at any time, using the “Erase all” button on the form.
